2024-25 Annual Back to School Forms

At the start of each school year, we ask families to complete the annual Annual Back to School Forms  for their current Selah students. This ensures we have the most up-to-date important student and family information including emergency contacts, student health & medication information, transportation needs, etc. We also provide you with important updates and information about some of our programs including Nutrition Services, Athletics and Transportation. 

Please have the Annual Back to School Forms completed for each of your students prior to the first day of school to ensure we have updated information before school begins.

The Annual Back to School Forms process for Selah School District current students is completed online through our Skyward Family Access system.

Translation Option:

  1. Use a Google Chrome Browser
  2. While on the application screen right click with the mouse
  3. Select “translate to”
  4. Click on the three dots
  5. Select “choose another language”
  6. Select the language you want to translate to in the drop down
  7. Select the “translate” button
